Please download and complete this form to write call options using your margin lending facility. The Call Options terms and conditions (PDF 285KB) are to be read and understood in conjunction with the BankSA Margin Lending Facility Agreement.

Asset Access Application (PDF,299KB)
Please complete this form if you wish to use equity available in your BankSA Portfolio Loan to support your margin loan. The Asset Access Terms and Conditions should be read in conjunction with the BankSA Margin Lending Facility Agreemnet.

Cash Management Account Form (PDF, 120KB)
Complete this form to authorise the opening of a Cash Management Trust Account (CMA). The CMA will be opened in the name of Value Nominees Pty Limited, a wholly owned subsidiary of Westpac Banking Corporation.

Complete and return this form if you wish to sell part or all your managed fund investment(s).

Drawdown Request (PDF, 68KB)
Use this form to arrange cash withdrawals from your Margin Lending facility.

Margin Lending Direct Debit Request (PDF, 113KB)
Debit your monthly interest charges directly from your bank account. In completing the direct debit form request you are accepting the terms and conditions of the Direct Debit Request Service Agreement (PDF, 56KB)

If you wish to fix your loan and prepay your interest complete and sign this form.

Tax File Number or Australian Business Number Form (PDF, 114KB)
Complete this form if you wish to provide your Tax File or Australian Business Number and have not already done so. Please note you do not have to provide these details but if you choose not to, tax may be withheld on any interest earned on your Cash Management Trust Account at the highest marginal rate plus the Medicare levy.
